  1. At what age am I covered / eligible a for Medicare Supplement?

  1. Most folks become eligible for Medicare benefits, and Medicare supplement policies, when they reach 65 years of age. Certain other factors, such as disability, may qualify a person for Medicare prior to reaching age 65.

 

  1. How do I submit a Medicare Supplement claim?

  1. To service a Medicare Supplement claim, all we need is the following:

A copy of your Medicare Explanation of Benefits form you received from Medicare ("EOMB") with your full name and Bankers policy number clearly written on it. Most people choose to write it on top, where there isn’t a lot of Medicare preprinted information. That’s all we need to process your claim!

  1. I haven’t received benefits on my recent Medicare Supplement claim. What should I do?

  1. Our claim service is one of the fastest in the industry. If you haven’t heard from us within two weeks of receiving your EOMB, it is because the claim never reached us. Just send us a copy of the EOMB and we’ll get right to work.

  1. How do I file a claim on my Hospital Confinement Indemnity policy?

  1. You need only send us a copy of the Hospital bills showing the dates of confinement and the diagnosis. Be sure to include your policy number to assure faster service.

 

  1. How do I file a claim on a Long Term Care or Home Health Care policy?

  1. We’ll need a completed claim form. In addition we need:
